WCS FinalForms

In order to be eligible to tryout and participate in athletic programs or activities, all required athletic annual physical forms must be completed electronically through the WCS FinalForms online dashboard. This enables parents and students to complete required athletic forms electronically. For parents and students new to Williamson County Schools, you can follow the link to the Final Forms page. Once there you will need to create a Final Forms account login using the same email that you used for your Skyward access. You will choose a new password, the login information is not automatically the same as your Skyward login information. Once you have chosen a password you will then be able to login and sign the forms for your student’s participation in Athletics. Coaches will communicate detailed information, including tryouts time/location, via FinalForms email. If you have trouble, contact Athletic Director,  Coach Longacre.

As Warrior athletes, students will make a commitment to themselves, teammates and the athletic program to follow the expectations of being an athlete. Woodland Middle offers a wide variety of sports in fall, winter, and spring seasons. Tryout Information

For the most current information on tryouts, please sign up for the sport(s) of interest in WCS FinalForms or download the Woodland Middle School App for iOS or Android. Coaches will communicate detailed information, including tryouts time/location, via FinalForms email and the WMS App. 
 
Woodland athletics competes under TSSAA guidelines. Any sports physical dated prior to April 15, 2026, will be considered expired on June 30, 2026. For an athlete to compete in school athletics during the 2026 - 2027 school year, sports physicals must be dated after April 15, 2026, and uploaded into WCS Final Forms.
